This is a most interesting paper, as its popularity amongst the BMC Public health readership attests. However, it would be very useful if the authors published the Stata programme files and associated data files which they used for this analysis, under the same Creative Commons license as the paper itself, so that others can more readily explore and extend the work and possibly translate the programmes to other epidemiological computing environments.
