Hypercholesterolemia TC ≥ 6.21 mmol/L or using lipid lowering drugs
Hypertension SBP ≥140 mmHg or DBP ≥90 mmHg or using any high blood pressure medications
Diabetes FPG ≥7 mmol/L, or 2 h-PG ≥11.1 mmol/L or taking anti-diabetic drugs
Low physical activity • Doing exercise or labor < 3 times a week (in first phase)
• Achieving < 600 MET (metabolic equivalent task)-minutes per week (in second phase)
Current smoker Participants who used cigarettes or other smoking implements daily, non-daily and occasionally at baseline
Education
 Illiterate/primary 0–5 years of schooling
 Cycle/diploma 6–12 years of schooling
 Higher than diploma > 12 years of schooling
 FH-CVD Prior diagnosis of CVD in a male first degree relative < 55 years or in a female first degree relative < 65 years
