  2. 2 The ORs within parentheses show ORs in comparison to the reference category 1.0 (for other diagnoses) for the categories of people that had received two and three diagnoses, to complement the ORs for the interaction terms since these are not interpretable without recalculation.
  3. 3 Adjusted for mortality and migration, i.e., those who died or moved away during the decade were subtracted from the total numbers.
