  1. The First tertile (lowest level of access) was compared to the last tertile (highest level of access). The highest level of access is referral
  2. Values ​​related to the second tertile are not shown in Table 4, as they did not remain significant in any of the regression models
  3. Variables that presented p-values up to 0.1 in the association analyses were included. The variables were entered into a model considering the dimensions (contextual, predisposing, enabling, and health needs; see Fig. 1). To build the final regression model, only the variables that were remained associated with the outcome in the previous blocks were included in the subsequent models
