Participant Group

Eligibility Criteria

Change Club Members

•Provide electronic informed consent

•Be willing to be randomized to either group

•Score “poor” or "intermediate" on at least one of the American Heart Association’s Life’s Simple 7 composite score items

•Live in one of the participating communities in New York or Texas

Friends and Family Members

•Provide electronic informed consent

•Be a friend or family member identified by a Change Club Member

Community Residents

•Provide electronic informed consent

•Live in one of the participating communities in New York or Texas

Extension Educators

•Provide electronic informed consent

•Serve as a Change Club leader

Participant Group

Exclusion Criteria

All Participants

•Cognitive impairment (if it precludes completion of assessments and/or intervention)

•Inability to communicate due to severe, uncorrectable hearing loss or speech disorder (if it precludes completion of assessments and/or intervention)

•Severe visual impairment (if it precludes completion of assessments and/or intervention)

•Inability to read (as it precludes completion of assessments and/or intervention)

•Already included in another study sample (e.g., Community Residents cannot also be Change Club Members)
