  1. aVaccination coverage data for the US and Puerto Rico were obtained from the National Immunization Survey (NIS) among children aged 19–35 months [11]. Birth dose coverage data for Brazil was obtained from the National Immunization Programme, Ministry of Health

Alberta, Saskatchewan, Manitoba, Ontario, Nova Scotia, Newfoundland and Labrador maintain a school-based immunization programme.
