  1. GIF, generalized impact fraction in percent; SI, 2.5 % to 97.5% simulation interval from 10,000 bootstrapped data sets.
  2. *Health Metric Score is defined as the number of ideal health levels for six cardiovascular disease risk factors.
  3. 1) Body Mass Index < 25 kg/m2.
  4. 2) Total cholesterol < 5.18 mmol/l (<200 mg/dl).
  5. 3) Systolic blood pressure < 120 mmHg and diastolic blood pressure < 80 mmHg.
  6. 4) Non-smokers.
  7. 5) Moderate physical activity ≥ 3 hours per week or vigorous physical activity ≥ 1 hours per week.
  8. 6) Non – Diabetes.
  9. †30% decrease in subjects with levels ≤3, with movement of this 30% to the group with ideal scores (≥4).
  10. ‡50% decrease in subjects with levels ≤3, with movement of this 50% to the group with ideal scores (≥4).
  11. §100% decrease in subjects with levels ≤3, with movement of this 100% to the group with ideal scores (≥4).
  12. ||The preventable number of MIs per 100,000 person-years.
