  1. †Co-morbidity 2004 & 2006 = coronary heart disease, diabetes mellitus, chronic obstructive pulmonary disease (COPD), transient ischaemic attack (TIA) or stroke, asthma, hypertension.
  2. Co-morbidity 2008 = As for 2006 co-morbidities + chronic kidney disease (CKD), Schizophrenia, bipolar disorder or other psychoses (“psychoses”).
