  1. a. Self-rated health reported as "excellent", "very good", or "good".
  2. b. Adjusted for age, sex, race/ethnicity, education, employment, and marital status.
  3. c. Adjusted for age, sex, race/ethnicity, education, employment, marital status, smoking, drinking, BMI, health care access, disability, history of any cardiovascular diseases (i.e., heart attack, angina, coronary heart disease, or stoke) and arthritis.
  4. d. Confidence interval.
  5. e. Age at diagnosis < 30 years and currently using insulin.
  6. f. Maximum subgroup sample size.
  7. g. Reported moderate or vigorous activity in episodes of at least 10 minutes, but did not accrue a combination of time equivalent to 150 minutes of moderate activity per week.
